Root Cause

Incident investigation and root cause analysis is a necessary tool to help ensure that there is not a recurrence of incidents that have taken place at work. To ensure a high quality analysis, companies must have the ability and the skill to do thorough investigations. With a thorough investigation, a root cause analysis can be completed for a particular incident. Root causes are the reasons why an incident has occurred, and with their identification, corrective actions can be put in place to prevent a similar incident from occurring.

ACIG Incident Investigation and Root Cause Analysis Training combines classroom instruction and group interaction allowing participants to gain the knowledge, skills and abilities necessary to perform root cause analysis, including incident investigation pre-planning, performing an incident investigation, developing  a timeline of actions, identifying the direct cause(s), analyzing for root cause(s) and developing corrective actions. Training is conducted over two days, and class size is limited 18 participants.
