Contractor Risk Profile Form

This is a contractor risk profile that ACIG originally developed in the early 1990's. It has been updated through the years. Filling out the form will help the contractor or agent/broker better understand the risks inherent in the contractor's operations. Some umbrella/excess carriers will accept this type of form in lieu of a traditional umbrella application.
